W-2 tax forms mailed; also available in C2HR

Human Resources mailed 2011 W-2 tax forms this week, so employees should check their mailboxes. The forms were sent to employees’ most current mailing addresses on file (typically a home address).

W-2 Tax Form Resources

For help understanding the W-2 tax form, see How to Read Your W-2 Form. Please note that, per IRS requirements, 403b amounts shown in Box 12 include only voluntary contributions to the plan and do not include mandatory contributions to your 403b.

If you need a replacement W-2 tax form, choose one of the following options:

  • New this year: Active employees can log into C2HR and click on the link for “Taxes” in the right sidebar. The online version of the W-2 tax form has the same information as the mailed W-2.
  • Call the Employee Service Center at (615) 343-7000. You can choose to have the replacement form mailed to the address on your W-2, or you can pick up the form at HR Express (you will need a photo ID).
