The Health Plus Wellness Credit designation survey has been sent by email to faculty and staff who are enrolled in the Vanderbilt Health Plan and have earned a Go for the Gold Wellness Credit for 2014.
Using this survey, faculty and staff may elect to apply their Go for the Gold credit toward:
- The Vanderbilt Health Plan account OR
- Membership at the Vanderbilt Recreation and Wellness Center.
The deadline to complete the Wellness Credit designation survey is Wednesday, Dec. 4, 2013.
The default, if no option is selected, will be the Vanderbilt Health Plan account.
