W-2 forms mailed; also available on C2HR

Check your mailbox this week for your 2012 W-2 form. Your form was sent to the most current mailing address on file with Human Resources (typically your home address).

New this year: The Affordable Care Act requires Vanderbilt to report the value of its health care plan on your 2012 W-2 form. You can find this dollar amount in Box 12, under Code DD. It includes the portion you pay and the portion Vanderbilt pays for your health coverage for 2012. The reporting is for informational purposes only and you are not taxed on this benefit.

Also note that, per IRS requirements, 403b retirement plan amounts shown in Box 12 include only voluntary contributions to the plan and do not include mandatory contributions to your 403b.

Replacement W-2 Forms

If you need a replacement W-2 form, choose one of the following options:

  • Active employees can log in to C2HR and click on the link for “Tax” in the right sidebar. The online version of the W-2 form has the same information as the mailed W-2.
  • Call the Employee Service Center at (615) 343-7000. To ensure confidentiality, HR does not fax W-2 forms. You can pick up the replacement in person at the HR Express Office with a picture ID or have the replacement mailed to the address printed on the W-2.
